The Leapfrog Group, a Washington, D.C.-based watchdog organization, publishes its biannual list of hospital safety grades each spring and fall for nearly 3,000 general hospitals, which are measured on criteria including the rates of preventable medical errors, accidents, injuries and infections, as well as patient experience.
